Yesterday we looked at the possible causes for the deaths of swimmers in Triathlon and other open water competition. Pre-existing cardiac conditions may be one plausible theory. One such pre-existing condition could be “Long QT syndrome.” This is an “inherited defect in heart rhythm that predisposes to syncope without warning (sudden fainting spells), dizziness, palpitations, seizures and sudden death. The name of the syndrome comes from the QT segment in the tracing on the electrocardiogram (ECG). This segment lasts slightly longer in the syndrome than normal. The heart takes longer to recharge itself between beats. Certain conditions can trigger an abnormal cardiac rhythm. Among the known triggers are intense physical exercise, swimming, being suddenly startled or badly frightened.

So, with swimming being a potential “trigger” and caffeine known to cause irregular heart beats (or “ectopic foci”), I looked for deaths associated with energy drink consumption and sport. According to the Sport Journal, when these are reported, they seem to be isolated cases involving multiple servings with high levels of caffeine. For example, according to Rice University over 15 years ago, one study looked at the whether caffeine intake increased the risk of heart disease. Over 45,000 people were evaluated and the data indicated a slightly increased risk of death from heart disease in individuals who had an average daily consumption of more than 5 cups of *decaffeinated* coffee.

As a trial lawyer who must present evidence with sufficient reliability to pass muster in court, the question becomes, “How can we know for sure if the combination of intense swimming, changing suddenly from a vertical position to a horizontal one, sudden increase in stress and anxiety, coupled with significant caffeine intake will lead to a cardio-pulmonary event or syncope ?” While testing on a swimming ergometer (VASA or similar equipment) with a sufficiently large number of subjects and placebos such as would yield statistically significant results, the fact that actual competition can occur in very cold or hot water and the athletes do not always get “air” every time they breath. The added stress of ingesting water during intense aquatic competition could be another piece of the puzzle. Certainly further study is warranted, and all athletes should have regular physical exams.

Truck vs. Bike – Guess who loses ? The YouTube from the DC biker’s helmetcam (Caution: Profanity) resonates with many of us who have ridden on DC metro area roads, commuted by bicycle and seen inexplicable road rage, distracted driving and other dangerous maneuvers by motorists. The troubling incident was discussed on the Landau Injury Law site.

I do not believe I have ever posted a comment to YouTube, but this video should be “Exhibit A” to those who contend bikers are not endangered by truck drivers or ridden off the road. After reviewing the film with my own bride (and bicycling partner for life) I wrote:

“With cars parked in the left lane, traffic relatively light in the other 2 lanes, I would have ridden a safe, straight line in the same situation. I have seen how devastating the injuries are from being “doored” & many motorists do not pay attention to bicycles riding inches from the curb. So when the biker is forced to go left to get around parked cars, they can get “clipped” by motorists paying more attention to cars on their left and straight ahead. If we want to curb unsafe conduct by people driving trucks, cars & motorcycles, then responsibility must be taken when they unnecessarily endanger others.”

P.S. If this had been a child riding to school & permanently injured, what would we make of the trucker’s cowardice then ?”

The National Football League is looking into kickoff modifications in order to help ‘deal with injury numbers’ according to news reports this week. Indicating that it was a matter of safety, along with possible suspensions for player who make illegal hits, the NFL is recommending that kickoffs be moved from the 30- to the 35-yard line — as it was before 1994. Also, kicking team members must line up within five yards of the ball. This would prevent the kicking team members from lining up 10-15 yards behind the ball to get a running start. By the time these offensive players got downfield, they were at full speed.

The sound of the impact when 300 pounds of professional football player, sprinting at top speed, strikes another player moving in the opposite direction at maximum velocity, it something not soon forgotten. The momentum of each player (speed times mass) is truly shocking. The league recognizes the potential for devastating and career ending head, neck and brain injuries and is looking into way to modify the game while still keeping its essential features. Read more at the Washington Examiner.

Sometimes even the fastest athletes need to stop and be thankful for the gifts and good health they have. Thanksgiving is a time for celebration. We celebrate family, friends and food. It is a unique holiday that does not memorialize the great deeds of some war hero or leader, but rather the bounties that we have in the United States and the American tradition of gathering around the table to give thanks.

It is also a time to think of those who may be less fortunate than ourselves. While I give of my time to volunteer, direct and help with school, sports and law related events, at ABRAMS LANDAU, we donate clothing, food, and supplies to the local free clinic, neighborhood resource center and community center. In addition, regular donations of goods to Purple Heart, AmVets, the Lupus Foundation, Salvation Army, and the Red Cross Blood drives and National Bone Marrow Registry collection efforts. While money may be tight throughout the USA, there are many ways we can give to others and contribute to help our fellow Americans. Everyone at the Landau Law Shop hopes to do even more in 2011.

Injuries to the brain and spine do not necessarily mean the end of exercise and sports. While brain injury can be a life changing event, advances in sports medicine and rehabilitation have enabled bike crash and car accident victims to remain more active than in the past. It is important to remain upbeat, active and exercise. As we used to say at ACSM (American College of Sports Medicine) meetings, “If it isn’t physical, it isn’t therapy !” Physical and occupational therapy is essential to maximum recovery after a spine or brain injury. Nerve cells need stimulation for recovery and repair. Reorganization may occur. Synapses, that is the contacts between the nerve cells, may strengthen with stimulation and use. Sports injury lawyer Doug Landau has several hemiparetic clients who are paralyzed on one side of their bodies. To advance recovery from hemiparesis after stroke, one novel treatment, Constraint-Induced Movement therapy (“CIMT”), entails the temporary restraint of the functional limb while the patient is encouraged to carry out tasks with the paralyzed limb. According to medical researchers, CIMT produces greater improvements in movement and functional use of impaired arms and hands.

The repair of broken connections between nerve cells is poorly understood. Doctors understand that a sudden snap of the head pushes the brain so forcefully inside the skull that the long processes of the nerve cells are severed. This effect is known as axonal shear. The Abrams Landau trial team has seen this in cases of head trauma or rapid deceleration car crashes. Contracoup injuries involves the stretching and shearing of the nerves in the brain, bleeding inside the skull and intracranial swelling. Extensive data has been gathered on the effects of ischemia, that is when the blood supply to the brain tissue is disrupted, and strategies are being developed to rescue nerve cells under this condition. For more on this interesting area of brain injury research and rehabilitation, click here.

Trampoline accidents and injuries can be catastrophic due to the impact and damage to the spinal cords and heads of victims young and old. The issues of adequate spotters, padding and instruction are usually at the forefront of any investigation into a trampoline accident. However, when the trampoline itself is defective, catastrophic injury can result regardless of the number of spotters, pads and training sessions. Herndon Virginia sports injury lawyer Doug Landau of Abrams Landau, Ltd. examined the dangers of such equipment over 25 years ago in his article for the Practicing Law Institute, “…With the Greatest of Ease – Gymnastic Products Liability” (Sports & Recreational Equipment, 1983). One of the reasons Abrams Landau supports the distribution of free bicycle helmets for children is to put safe, non-defective sports equipment in play so no one gets hurt.

A California sporting goods company has recently announced a recall 160,000 sporting trampolines over the risk of the structures breaking. Bravo Sports announced the recall in conjunction with the Consumer Product Safety Commission last week, saying “the trampolines, if improperly assembled, can result in weak support, posing the risk of the top rails or legs bending or breaking during normal use.” The trampolines were imported from China by Bravo Sports of Santa Fe Springs, Calif., and sold nationwide from January 2007 through September 2010 for between $200 and $400. According to the news report, the company has received 247 reports of problems with the trampoline railings.

The trampolines, if improperly assembled, can result in weak support, posing the risk of the top rails or legs bending or breaking during normal use. The firm has received 247 reports of problems with the top rail, the commission said in a statement. The recall involves AirZone and Variflex trampolines. They were sold in 12-feet, 13-feet and 14-feet models in blue, yellow and red. Consumers were advised to stop using the trampolines and contact Bravo Sports for instructions on how to inspect and reassemble the support pieces. Any top rails and legs damaged due to assembly errors would be replace for free, the commission said. Consumers can call 877-500-2459 for information.

The recent in Alexandria underscores the importance of drivers looking BOTH ways when crossing DC area cycling, skating and running paths. Too often motorists are looking to see vehicular traffic approaching in whatever direction is closest to their car or truck. Northern Virginia drivers tend to pull out to the street without regard for cyclists or joggers. The “Share the Road” goal is observed more in the breach of this safety maxim.

In an area blessed by so many bike trails and cycling venues, it is incumbent on school and private drivers education professionals to teach students how to safely share the road. With an increase in the number of bike commuters and recreational cyclists, habits need to change to recognize that not looking out for bikers and runners can lead to injury and death. Cell phone use has only exacerbated the problem, as more drivers are driving “distracted.” Distracted drivers are not focusing on the other travelers around them. When a bicycle rider or jogger is traveling from one side or the another and the motorist is focused only on the road ahead, their GPS and/or their cell phone, impact, injury and disability are usually the tragic result. When traveling with friends and family in a car, remind them to look out for cyclists, runners and others on the road. Together we may be able to change dangerous habits and increase the safety of everybody on the roads and trails that surround our Nation’s Capitol.

John Brannock, a former training partner and team mate, came in this weekend on a business trip. As is Doug Landau’s habit, the Herndon trial lawyer convinced his friend to join him for the first annual Warrenton Sprint Triathlon on Sunday. Landau borrowed a bike, brought extra helmets and other safety gear, and loaded the mini van so that they could car pool to the pool. The 300 yard indoor pool swim, 11.8 mile hilly bike ride and the 5 km. up and down run course would take less than 2 hours. However, Brannock had been in a mountain bike accident on Friday, and his bruises were just coming into their full black and blue color ! The new Warrenton Aquatics Center (“WAC”) was a clean, state of the art facility for this new Virginia Triathlon Series event in Fauquier County. Located just passed the exit for “Old Town” Warrenton and the Court House area where the ABRAMS LANDAU Law firm has represented injured workers, car crash survivors and the families of brain injured victims, the bike course takes races on Route 29/211 and the run course is a very challenging two loops. It was only after the race that Brannock showed Lawyer Landau his cuts, bruises and swelling ! Brannock raced on a borrowed bike, ran with the wrong shoes and was limited by his battered joints. Nevertheless, despite these handicaps, and the 2 hours time differential coming from his home in Colorado, John had a strong showing and even won a prize for having come to Fauquier County from the mid-west ! Congratulations John Brannock !
